Please click on each link and download the forms. After downloading each form, please read them carefully there's a lot of information. The final step will be to complete the Volunteer Application and Class Vision Card (if you are teaching a class) and return them via email to admin@epicenterarts.com.
The board will review the application and contact you as soon as possible.
